Adult Transition Team
If you need support as an adult a specialist transition social worker from the Complex and Inclusion team will attend your annual review meetings at school from year 9
They will help you to plan for your future
You will be helped to think about what you want to do when you leave school

Working together for Change, Dudley Parent and Carer Forum
The Dudley Parent Carer Forum are a group of parent carers
They look after children and young people aged 0-25 who have disabilities and additional needs
They meet to share their thoughts and concerns
They empower parents and carers to have their voices heard
The forum is supported by staff from Dudley CVS
The forum is endorsed by DMBC
The forum works closely with clinical commissioning group who are supporting the development of the forum
